San Francisco is a city and county in Northern California and is the 13th most populous city in the United States and the 4th most populous in the state. The area makes up the San Francisco Peninsula and San Francisco Bay Area. It is estimated that there are more drug addicts in San Francisco then there are students enrolled in the local high schools. Homeless and addiction are a growing problem in the city leading to a variety of issues such as $54 million spent on street cleaning in 2018. There is an estimated population of around 25,000 drug users, which is about 522 drug users per square mile in San Francisco. Per an article outlining the drug problem, written by the Hoover Institution, the drug abuse problem has become a public health crisis. The city gave out over 5.8 million hypodermic needles in 2018 and collected over 3.8 million dirty needles.

The latest project in the city is to spend $50 million on programs based on harm reduction with the intent to help 4000 addicts with behavioral health treatment and housing assistance. According to the California Health Care Foundation (CHCF), within the Greater Bay Area, there are 2.2 residential beds per 10,000 adult population and 1.9 residential detox beds per 10,000 population. As of 2016, the state of California had 26 hospitals with 771 beds licensed to provide inpatient treatment services, only located in 10 counties.
