Aspire Health Partners - H.O.M.E.: Helping Others Make the Effort

This facility is serving families in Osceola to break the cycle of homelessness by offering housing and support facilities. Our mission at HOME is to eradicate the dependency on others by offering many programs. The services offered at Aspire Health Partner include alcohol and substance abuse, mental health, prevention, education and homeless services. We have an intensive screening before enrollment in our program. Any family in need of assistance needs to set up an appointment with HOMELife. 

Drug and alcohol addiction is by no means an easy or simple problem to get rid of.  This is an issue and a crisis that just tends to get worse and worse as the years go by.  Hardship and crisis one and the same, addiction is a problem that needs professional assistance to get rid of it.  The Aspire Health Partners H.O.M.E.: Helping Others Make the Effort is one such program that is very engaging and helpful.

Helping Others Make the Effort (HOME) is working to break the cycle of homelessness by providing housing and supportive services to homeless families. HOME offers several program options, dependent on the individual family's need and housing urgency.

HOMELite- HOMELite is the entry point to all of HOME's programs. It includes outreach , intake, information at and referral and case management. When a family first contacts HOME for service, a fine to ten minute screening is used to determine if the family qualifies for HOME services or needs a referral to another agency. IF a family can be helped by HOME, an appointment will be set for them to come to the office for a more in-depth assessment of needs. HOMELite provides assistance with making application for food stamps, case management and life skills education.
