House of Change Behavioral Health Center is a community-based outpatient addiction treatment provider in Baltimore, Maryland. The center offers evidence-based behavioral health services for adult men and women aged 18 and older, with programming that spans partial hospitalization, intensive outpatient, and traditional outpatient levels of care. House of Change also provides community residential housing to support clients in maintaining sobriety while they participate in outpatient treatment.
Outpatient Treatment and Partial Hospitalization
Treatment at House of Change is guided by individualized, person-centered plans and includes individual counseling, daily group counseling, family counseling, relapse prevention strategies, life-skills training, crisis contingency planning, and disease management education. The partial hospitalization program adds occupational therapy, vocational therapy, recreational therapy, communication and social skills training, and service coordination and referral to the core clinical services. Clients entering at the intensive outpatient level receive case management, referrals to outside services, and ongoing assessment, and may step to traditional outpatient care as they progress. Eligibility is based on a primary substance use disorder diagnosis and an ASAM assessment for appropriate level of care.
Community Residential Program and Support Services
The Community Residential Program provides a structured sober living environment for clients who need housing stability alongside their outpatient treatment. Residents receive comprehensive assessment, case management, individual counseling at least monthly, group counseling focused on relapse prevention, leisure and recreational activities, toxicology screening, and life-skills support, while participating in a minimum of five hours of outpatient treatment services. House of Change also partners with the City of Baltimore, Abel Foundation, and Loyola University Maryland to connect clients with community-based care, workforce development, and family outreach services aimed at reducing recidivism.
